You will need the Following:

Current Picture I.D.

Social Security Card (if applicable)

Proof of income for all individuals in the household for the past 4 weeks. If no income, bring a statement from whoever is helping you get by.

Food Stamp letter (if applicable)

Disability/Social Security/SSI award letter (if applicable)

List of all medications you take with doses
-
Denial statements from SS, SSI, Disability, Unemployment, Food Stamps, Medicaid or Medicare (if applicable)
Requirements

Must be a Spartanburg County resident

Must NOT have any health insurance, such as Medicaid, Medicare, or VA benefits

Must meet income guidelines. Income means ALL income for ALL family members living in the household.

Making An Appointment

If you are a current patient and need to make an appointment to see a doctor, call (864) 542-CARE, Ext. 21.

Note: If you leave a message for a staff member, please leave one message and allow 48 hours for a response.
Medication Refills PLEASE READ

Please not wait until the day of or when you run completely out of meds before you call for prescriptions. They should be called in a week before at least. For Wellvista medications there are reminders on the bottle for when you need to call for refills.

Monday - Thursday: 9:00 AM to 5:00 PM
Friday: 9:00 AM to 3:00 PM
Pharmacy Hours
Monday: 9 AM- 4 PM
Tuesday: 1 PM- 8 PM
Wednesday and Thursday: 10 AM to 4 PM
Friday: 9 am-1 pm
**Pharmacy hours vary from normal clinic hours and may vary in Summer - patient text messages will be sent out with information on any change in schedule.
