HOW TO APPLY:

You must be a resident of Spartanburg County and have no medical coverage of any kind.

Applications are taken in the lobby:

Mondays – 10 am to 1 pm

Thursdays – 1 pm to 4 pm

Please bring the following:

· Picture I.D. with current address

· Social Security card (if available)

· Proof of income for all individuals in household for the past 4 weeks

· If no income bring a statement from whomever is assisting/helping you get by

· Food Stamp letter (if applicable)

· Disability/Social Security/SSI award letter (if applicable)

DONOR INFORMATION

St. Luke’s Free Medical Clinic is a 501(c)3 tax exempt organization; all donations are tax deductable. St. Luke’s receives no direct funding from any government source. It relies on donations, grants, and the community at large to meet an annual operating budget of $600,000.
